allow an optional 'start' vector; this will have to include the coefficient vector (determined by the columns of the model frame/design matrix) as well as any relevant auxiliary variables (i.e. ZI parameter for zero-inflated models, theta for NB models) -- and possibly random effects variances? Perhaps the coefficients should be specified as a list of vectors ("fixef","varcoefs","extra") ?
